Age Proof for LIC Insurance Policy (Standard and Non-Standard)
The following certificates are generally accepted as standard age proof
- School or College Certificate or authenticated extract from the school or college records, if date of birth is mentioned
- Passport (Whole Self) issued by passport authority of India.
- Identity Cards issued by govt, quasi govt, provided DOB is mentioned
- Certificate extract from service register in-case of
(1)Government and quasi government employees
(2)Commercial institutions and industrial undertakings including public limited companies, duly countersigned by the proposer provided, age must have been verified by the employer
- Certified extract from the municipal or other records made at the time of birth
- Certificate of baptism or certified extract from the family Bible, if it contains the DOB
- Identity cards issued by the defense department, in case of defense personnel, showing the DOB or age
- Marriage Certificate in case of roman Catholics issued by Roman Catholic Church
- Domicile Certificate in which the date of birth stated was proved on the basis of one of the standard age proof accepted to LIC
- Horoscopes maintained by a Hindu Family in a Bahi or family horoscope book can be considered as standard age proof provided Manager (NB) is satisfied with its reliability and originality

LIC accepts following Non-Standard age proofs (NSAP) if the proposer does not have any of the standard age proof
NSAP is classified into 3 categories: NSAP 1, 2 and 3
NSAP-1: includes
(1)PAN Card issued by income tax department
(2)Driving license issued by RTO
NSAP-2: includes
(1)ESIS Card
(2)Marriage certificate in-case of Muslims
(3)Service record where age is not verified at the time of entry into service or where only year of birth is mentioned
NSAP-3: includes
(1)Horoscope other than that maintained by a Hindu family in a Bahi or Family horoscope book
(2)Elder's Declaration (Form no 3261)
(3)Self Declaration (Form no 5220-stamped)
(4)Self Declaration (Form no 5096-stamped)
(5)Certificate issued by village panchayat
(6)Any other certificate not classified as standard above
(7)Ration card (Issue date clearly mentioned).
(8)Election voter card only if date of issue is mentioned
